The following is a list of resources that we have devised to offer assistance and support to you along your journey. These website addresses include areas such as disability and Medicaid, transportation, assistance with medications, and links to other related resources. This information and an extensive list of other resources are available in written format at all of our locations, please ask the office for an ACC Resources 2008 booklet. For additional help or information, please contact your oncology social worker.

For assistance with questions about Medicare, Medicaid, or Social Security Disability

To begin the application process for social security disability (SSDI), call 1 (800) 772-1213 between 7:00 AM and 7:00 PM, or you can go to their website to file your application online (www.ssa.gov). To begin the application process for Medicaid, contact your county Department of Family and Children Services for an application, http://dfcs.dhr.georgia.gov.

For assistance with medications

For persons who do not have prescription drug coverage, please visit www.needymeds.com for a listing of generic and brand name drugs that are available through pharmaceutical patient assistance programs. This will provide an explanation of the application process and in most cases, you can download the application itself.  Fill out the application in full, except for the Physician Section, and take it to the prescribing doctor for submission to the drug company. Other similar websites include www.helpingpatients.org or www.rxhope.com or www.rxassist.org.

If you don’t qualify for the traditional Patient Assistance Programs, you may be able to qualify for a drug discount:

Rx Outreach - More than 125 different medications in over 45 different therapeutic categories totaling more than 312 medications. Offers 90-day supply for $20, $30 or $40 co-pay. Household income restrictions apply. For more information call: (800) 769-3880 or visit www.rxoutreach.com

Together Rx Access - Discount on select medications for persons who are without prescription drug coverage and meet income guidelines. For more information call: visit www.togetherrxaccess.com

Parnership for Prescription Assistance – The Partnership for Prescription Assistance brings together America’s pharmaceutical companies, doctors, other health care providers, patient advocacy organizations and community groups to help qualifying patients who lack prescription coverage get the medicines they need through the public or private program that’s right for them. For assistance with transportation to and from doctor’s appointments

For non-emergency medical transportation for Medicaid patients, please call Metrotrans/Southeastrans, Inc. at (770) 693-8401. They require at least 3 days notice for routine transportation needs.

American Cancer Society’s Road to Recovery  (888) 227-6333      

ACS may assist with transportation to and from chemotherapy and radiation appointments, this program is based on volunteer driver’s availability.  For persons who have their own transportation to/from cancer related treatment, ACS may provide gas card assistance to help defray transportation expenses.

Transportation may be available through the county-based aging services programs, to find the number for your county, call Agewise Connection (404) 463-3333. Agewise Connection is a user-friendly telephone service offering consultation, assessment of client needs, referral services and useful information pertaining to aging services and programs.  Some of the most common services include: adult day care, financial assistance, care management, community care services, transportation services, home delivered meals (Meals on Wheels), in-home health / custodial care, and personal care homes and housing options.

Georgia Crisis & Access Line

Twenty-four hour crisis and suicide hotline, also serves as a Statewide single point of contact for accessing mental health, addiction, developmental and behavioral health crisis services throughout the State of Georgia.

Georgia Senior Legal Helpline

The GSLH is a telephone service staffed by attorneys who take calls from older Georgians and their families.  Hotline attorneys give legal advice, brief service and referrals to other attorneys or services where necessary. The Hotline is free to Georgians 60 years and older.
